Financial Assistance
Heart of Texas Community Health Center (HOTCHC) (dba Family Health Center) offers financial screening services to all patients. Screening for financial assistance through the HOTCHC Eligibility Department is offered for various financial assistance programs (see information below).

Financial Assistance Programs:
GOOD HEALTH CARD (GHC): The program provides discounts for medical, dental, and pharmaceutical services. Discounts range from 50% - 100% Restrictions apply.
TITLE V: This program has been designed to provide prenatal & dental care for women and children who do not have medical coverage.
FAMILY PLANNING: A state funded prograni that assists men & women with faniily planning services.
Assistance programs are valid only at Family Health Center pharmacy and clinics WE CAN ALSO HELP YOU APPLY FOR: Medicaid, CHIP Perinatal, SNAP, TANF, Healthy Texas Women, CHIP, Medicare Part D
