This is a qualifying program only

MUST BRING ITEMS:

Birth certificate of the child
Social Security Card of the child
Parent/Guardian Picture ID
Current Shot record
Current Proof of Residence (must have your name and physical address, and date listed on the bill. Proof of residence accepted are lights, water, gas, lease agreement, and mortgage)
Proof of income (food stamp letter with your name, children and has your eligibility group number listed, OR last income tax filed, OR current month worth of check stubs.
