--------------------------------------------------------------------------------------------------------------------------------------------------------- CADET SPORTS MEDICINE 2013-2014 Athletic Physicals ALL ATHLETES MUST HAVE A NEW PHYSICAL EACH YEAR. Athletic physicals are valid for one academic year; however, they may not be dated prior to May 1, 2013 in order to be valid for the 2013-2014 school year. CISD athletic pre-participation packets for the 2013-2014 school year are now available for downloading. All athletes must have a new packet on-file prior to any athletic participation. All pre-participation physicals must be completed on the forms within the physical packet; any other forms will not be accepted. The Connaly ISD Athletic Department will be offering Athletic Physical Examinations for all students who are planning to be in athletics and other extracurricular activities requiring a pre-participation examination, during the 2013-2014 school year. Physicals will be offered at the following times: High School-Tuesday May 7th at 6pm-High School Gym Junior High School-Tuesday may 14th at 6pm-Junior High Gym In order for your child to have their physical performed during these times they must have all of the following completed and in their possession whey they arrive: -Connally ISD Athletic Physical packet with all blanks completed -Money for Physical (Cost is $15, cash only) Physicals not performed at the school must be performed by a licensed Physician, Physicians assistant, or Nurse Practitioner and turned into the Connally ISD Athletic Department. Properly completed physical and all other required forms must be turned in prior to the student being allowed to participate in any athletic conditioning, practice, or competition at Connally ISD. All pre-participation physicals must be completed on the forms within the physical packet; any other forms will not be accepted. 2013-2014 Pre-participation packets can be printed from www.connally.org/Page/579 or Physical packets can be picked up from the Athletic Trainer or coach of our campus.
